Physicals are required yearly for marching band students. These physicals are also good for all the school sports for the entire year.

     This information is to be completed by parent(s) or guardian(s) of each marching band student. Please make sure that we have updated information on file.

This form is in addition to the general medical form that is kept on file with the Band Director. Please complete this form ONLY IF your student has any medical issue that we need to be aware of and/or is taking any medication.

     This form is in the handbook and must be signed by parent(s) of all band students before they can be transported to any events by Gwinnett County buses, a Shiloh High School Band staff member, designated chaperone or another student or parent.
